Use the form below to request and appointment. Your doctor (or other referrer) may have sent us their questions, we believe that its important that you get the opportunity to explore your questions and concerns. That way, even if we can't answer them on the spot, we can make sure we do the appropriate tests and that the report covers them. Some common ones are:
- Is my memory worse than other people my age?
- Why am I having so much trouble concentrating at work since my injury?
- What can I do about these problems?
Help us prepare to meet you
A lot of the tests feel a bit like going back to school. For some people who come to see us, its been quite a few years since they were there, and they are a bit rusty! Don't worry, we understand. You are not expected to get everything right, just do your best. You may find that you are better at some things and not so good at others, that is okay.
It helps if we have a bit of information about you before you attend, so please fill this in and submit it!
Time involved
We allow enough time to understand your difficulties and the impact they have. The tests take several hours, so your appointment can be up to four - six hours long. However, we do take breaks along the way. We've compiled a handy checklist to make sure that you remember everything you will need!
